Release of Information

Most laboratory results are available within 10-14 business days.  You must be able to verify your identity in order to receive results over the telephone.  If you have a primary care provider and you have signed a release of information for that person we can fax the results to their medical office. If you do not have a primary care provider you may arrange an appointment with one of our medical or nursing staff and review the results in our office. If the medical assessment was performed in an emergency department call 425-297-5776 during regular business hours for results.  If the medical assessment was performed in the Intervention Center Clinic call 425-297-5776 during regular business hours for results.

During the course of an investigation, medical records are only released to the investigating agency (law enforcement, Child Protective Services, Adult Protective Services, Attorney General, etc).  Following completion of the investigation, medical records may be released to parents or legal guardians.  In order to collect the records, you must sign a release of information form and present with proof of identification.  If you are a parent or legal guardian for a child or vulnerable/dependent adult, you must bring identification (birth certificate showing you as parent, guardianship papers, or other legal documents granting you the right to the medical records) before the records can be released.

If you need to present proof of the medical assessment to another organization or the courts, a declaration can be provided for you.  The above identification stipulations apply.
